Community First Choice Services
At Minders Connect, we believe that every individual deserves to live with dignity, independence, and comfort in their own home. The Community First Choice (CFC) Program is designed to support older adults and individuals with disabilities so they can remain active and cared for in their community, rather than in a nursing facility.
Through CFC, families can access:
- Personal care assistance – help with bathing, dressing, eating, and other daily needs
- Homemaker services – meal preparation, light cleaning, and laundry
- Additional supports – emergency response systems, home safety modifications, and meal delivery
- Unlike traditional waiver programs that often have waiting lists, CFC services are offered directly through Medicaid with no enrollment limits.
- That means eligible families can receive the care and support they need without delay.
