Our Approach

Initial intake Assessment / Service Starts

1) Schedule intake session with parents and request child's required documents.
2) Conduct initial assesment to identify child's goal and needs.
3) Caregiver interview and selection.
4) Discuss child's schedule.
5) Sign consent form.
6) Service commence.

Orientation Phase / Develop & Submit Treatment Plan (within 30 days of intake).

1) Training provided to Technician on specific needs of child Technician works on buildingrapport with child and vice versa. (Transportation tocommunity is not advised within the first 2 weeks of service).
2) Obtaining baseline data while continuing to work with child.
3) Develop Draft Treatment Plan (TP).
4) Review TP with Parents and obtain Parent’s signature.
5) TP submitted to Service Coordinator, within 30 days from when service commences.

Implement Treatment Plan

1) Provide intervention to address identified goals for child.
2) Electronic documentation of child’s daily progress by Technician.
3)Supervision of Technician by Supervisor.

Monitoring, Communication & Periodic Progress Report

1) Documentation of child’s progress.
2) Continuous monitoring of Technician’s by Supervisor.
3) Monthly Feedback from parents.
4) Six (6) Months Progress Report.
5) Unusual Occurrence & Incident Report, as required.
6) Parent’s access to child’s schedule; status report on service hours services used and balance hours.
7) Periodic Newsletters.
